AbstractObjectiveTo analyse the association between family structure and adiposity in children.DesignCross-sectional and longitudinal analysis of the IDEFICS (Identification and prevention of dietary- and lifestyle-induced health effects in children and infants) study cohort.SettingPrimary schools and kindergartens.SubjectsChildren (n12 350; aged 7·9 (sd1·8) years) for the cross-sectional analysis and children (n5236; at baseline: normal weight, aged 5·9 (sd1·8) years) for the longitudinal study underwent anthropometry. Family structure was analysed as (i) number and type of cohabiting adults and (ii) number of siblings.ResultsIn the cross-sectional analysis, after controlling for covariates, children living with grandparents had significantly higher BMIZ-score than those living with both parents (0·63; 95 % CI 0·33, 0·92v. 0·19; 95 % CI 0·17, 0·22;P< 0·01); in addition, the higher the number of siblings, the lower the BMIZ-score (only child = 0·31; 95 % CI 0·24, 0·38; 1 sibling = 0·19; 95 % CI 0·16, 0·23; 2 siblings = 0·15; 95 % CI 0·09, 0·20; >2 siblings = 0·07, 95 % CI 0·04, 0·19;P< 0·001). Over the 2-year follow-up, differences in weight gain were observed across family-structure categories. Further, the risk of incidence of overweight/obesity was significantly lower the higher the number of siblings living in the household (v. only child: 1 sibling = 0·74, 95 % CI 0·57, 0·96; 2 siblings = 0·63, 95 % CI 0·45, 0·88; >2 siblings = 0·40, 95 % CI 0·21, 0·77), independently of confounders.ConclusionsThe study suggests that an independent association between family structure and childhood obesity exists.

In analyzing firm entry and exit across Belgian manufacturing industries, this paper presents evidence that import competition and foreign direct investment discourage entry and stimulate exit of domestic entrepreneurs. These results are in line with theoretical occupational choice models that predict foreign direct investment would crowd out domestic entrepreneurs through their selections in product and labor markets. However, the empirical results also suggest that this crowding out effect may be moderated or even reversed in the long-run due to the long term positive effects of FDI on domestic entrpreneurship as a result of learning, demonstration, networking and linkage effects between foreign and domestic firms.

Abstract The longevity and high activity of the cork cambium (or phellogen) from Quercus suber L. (cork oak) are the cornerstones for the sustainable exploitation of a unique raw material. Cork oak is a symbolic model to study cork development and cell wall suberization, yet most genetic and molecular studies on these topics have targeted other model plants. In this study, we explored the potential of taproots as a model system to study phellem development and suberization in cork oak, thereby avoiding the time constraints imposed when studying whole plants. In roots, suberin deposition is found in mature endodermis cells during primary development and in phellem cells during secondary development. By investigating the spatiotemporal characteristics of both endodermis and phellem suberization in young seedling taproots, we demonstrated that secondary growth and phellogen activity are initiated very early in cork oak taproots (approx. 8 days after sowing). We further compared the transcriptomic profile of root segments undergoing primary (PD) and secondary development (SD) and identified multiple candidate genes with predicted roles in cell wall modifications, mainly lignification and suberization, in addition to several regulatory genes, particularly transcription factor- and hormone-related genes. Our results indicate that the molecular regulation of suberization and secondary development in cork oak roots is relatively conserved with other species. The provided morphological characterization creates new opportunities to allow a faster assessment of phellogen activity (as compared with studies using stem tissues) and to tackle fundamental questions regarding its regulation.

This paper focuses on the intergenerational transmission of age at first marriage from mothers to daughters in rural Holland before and during the early stages of the fertility transition. We use a detailed dataset with two generations of marriages stemming from the province of North-Holland in the Netherlands. Multilevel models are used to analyse the effect of first generation family characteristics on second generation daughters' ages at first marriage. A crucial advantage compared to conventional family reconstitution studies is that we traced daughters wherever they migrated in the Netherlands. The results clearly indicate that age at first marriage was to some extent inherited from mothers to daughters. Yet, the inheritance effect is not common to all social classes and religious denominations. It is present among the working and middle classes but virtually absent among farmers. It is strong among Protestants but weak among Catholics. These findings support suggestions in the recent literature that family inheritance of reproductive behaviour is stronger in societal circumstances with less group pressure and more individual decision-making.

This article investigates the responses of intersectional activists to the political opportunity spaces made for them by international organisations at the European transnational level. To do so, it uses Romani women’s activism and the Council of Europe as case studies, and develops a heuristic tool – the (Un)Safeness Response Line – that builds on five categories of analysis: compliance, compromise, contestation from within, opposition and denial. Mostly based on semi-structured and unstructured expert interviews conducted in 2016 and 2017, this study observes that Romnja activists have been mostly compliant with the political agendas of international organisations. Yet, they have not always perceived the opportunity spaces created for them as safe spaces. This has manifested in negotiating safeness and accepting compromises, as well as in strong opposition. This article shows that opportunity spaces for intersectional activisms are not enough, and that the quality of such spaces matters. Thus, it fosters a more critical approach to the study of contemporary social movements.

Abstract Objective Literature indicates a decline in patient-centredness in medical students, especially during clinical clerkships. We examined the impact of preclinical communication skills training (CST) on students’ development of patient-centred attitudes and attitudes toward CST during clerkships. Methods We prospectively compared two cohorts before and after clerkships: one cohort (n = 48) had not received CST, whereas the other (n = 37) had received a five-year CST. We assessed the impact using five validated questionnaires. Results Communication trained students slightly but significantly declined in patient-centred attitudes (3/4 scales) and attitudes toward CST during clerkships, whereas the scores of the untrained students remained stable (5/5 scales). Both cohorts did not differ in attitudes before clerkships. In the trained cohort, males mostly showed a sharper decline than females. In the total group (n = 85), females demonstrated higher attitude scores toward CST, and in 1/4 scales measuring patient-centred attitudes. Conclusion This cohort study suggests that CST might make students more vulnerable to decline in attitude scores during clerkships. Practice implications These remarkable findings, contrary to what educators would expect to result from their efforts, challenge medical education to address the new questions that are raised about the validity of the questionnaires, the impact of CST and the learning processes involved in the development of patient-centredness.

Abstract The cystic fibrosis transmembrane conductance regulator (CFTR) anion channel is essential to maintain fluid homeostasis in key organs such as the lungs or the digestive systems. Functional impairment of CFTR due to mutation in the cftr gene lead to Cystic Fibrosis (CF) the most common lethal genetic disorder. Here we observe that the first nucleotide-binding domain (NBD1) of CFTR can spontaneously adopt an alternative conformation that departs from the canonical NBD fold previously observed for CFTR and other ATP-binding cassette (ABC) transporter proteins. Crystallography studies reveal that this conformation involves a topological reorganization of the β-subdomain of NBD1. This alternative state is adopted by wild-type CFTR in cells, where it leads to enhanced channel activity. Single-molecule fluorescence resonance energy transfer microscopy shows that the equilibrium between the conformations is regulated by ATP binding. However, under destabilizing conditions, such as a the prominent disease-causing mutation F508el , this conformational flexibility enables unfolding of the β-subdomain. Our data indicate that in wild-type CFTR switching to this topologically-swapped conformation of NBD1 regulates channel function, but, in the presence of the F508del mutation, it allows domain misfolding and subsequent protein degradation. Our work provides a framework to design conformation-specific therapeutics to prevent noxious transitions.

This is the first part of a scientific statement from the Heart Failure Association (HFA) of the European Society of Cardiology focused upon the pathophysiology of Takotsubo syndrome and is complimentary to the previous HFA position statement on Takotsubo syndrome which focused upon clinical management. In part 1 we provide an overview of the pathophysiology of Takotsubo syndrome and fundamental questions to consider. We then review and discuss the central role of catecholamines and the sympathetic nervous system in the pathophysiology, and the direct effects of high surges in catecholamines upon myocardial biology including β‐adrenergic receptor signalling, G‐protein coupled receptor kinases, cardiomyocyte calcium physiology, myofilament physiology, cardiomyocyte gene expression, myocardial electrophysiology and arrhythmogenicity, myocardial inflammation, metabolism and energetics. The integrated effects upon ventricular haemodynamics are discussed and integrated into the pathophysiological model.
